There are two ways to sign-up:
1) Come to sign-ups on October 23rd or November 8th between 6:00 and 9:00pm at the recreation
building on Cottage Street
2) Download registration form and mail completed form along with payment to:

Make checks payable to: Bridgewater Youth Lacrosse (NOT US Lacrosse)
Do not mail form to US lacrosse, we will mail all forms to them. All US Lacrosse fees are included in registration fees.
Fees:
U11 thru U15:
$170.00 per child, families with multiple children in the program can contact the league for rates.
U9:
$80.00 per child.
Friday Night LAX Indoor League:
$85.00 per child.
All registrations must be in by December 1st, after December 1st there will be a $25.00 late fee.
Due to the need to purchase equipment and uniform there will be NO refunds after January 1st.
